Product Safety Recall - STOP USE NOTICE - Guardian Self-Retracting Lifelines (SRL's)

"As you may all be aware, there were several safety recalls for Self-Retracting Lifelines (SRL’s) issued this summer by different manufacturers. Guardian has recently issued a STOP USE NOTICE recall information regarding the Guardian Fall Protection and Web Devices 3-Way Rescue and Retrieval self-retracting lifeline. Under certain conditions, braking forces may cause gear misalignment, potentially resulting in diminished rescue/retrieval functionality.

THERE HAVE BEEN NO ACCIDENTS OR INJURIES RELATED TO THIS PRODUCT RECALL.

Please reference the attached PDF file.

Please review your equipment inventory for these devices and take any necessary action as noted by the manufacturer."

3M Recalls Self Retracting Lifelines (7/27/19 Email)

Region 2 VPP Sites:

"FYSA – 3M Fall Protection announces an immediate stop use and product recall of the 3M™ DBI-SALA® Twin-Leg Nano-Lok™ edge and the Twin-Leg Nano-Lok™ Wrap Back Self-Retracting Lifeline. 3M has determined that in a fall and under certain conditions, the energy absorber of these devices may not properly deploy which could expose the worker to serious injury or death. Users/owners should stop using affected Nano-Loks immediately and take them out of service. For more information, please see the link below:

MANDATORY STOP USE NOTICE (8/28/19 Email)

Region 2 VPP Sites:

FYI/A – Honeywell has issued a Stop Use Notice to inform customers of potentially non-conforming brake components in certain models of Miller MightEvac and MightyLite Self-Retracting Lifelines (SRLs) ranges manufactured within a specific timeframe. While there have been no reported incidents due to this nonconformity, continued use of the product could result in serious injury or death. Please see the link below for more information and take immediate action if your equipment inventory is included in the recall.
